"Extra Mile" Award Instructions & Nomination Procedure
Instructions:
- All nominating organizations should appoint a representative to work with the nominee; this will help ensure an accurate and high quality nomination for submission. The quality of the nomination will play an important part in determining recommendations by the selection committee.
- Only complete nominations (see below) should be mailed to The Alliance for Youth, 1907 Ascension, Suite 410, Arlington, TX, 76010, prior to March 31, 2008.
- All nominees must confirm in their nomination that they will attend the luncheon.
Complete applications must include:
- A nominating letter which includes the following paragraphs:
- Servant profile (500 words max). Provide examples of how the nominee demonstrated the value of servitude:
- Going the extra mile when helping youth
- Willingness to listen
- Ability to empathize
- Desire to help youth see the big picture
- Searching for ways to help youth heal and grow
- Persuasive leadership that shows youth the way
- Leader profile (500 words max). Provide examples of how the nominee led community efforts:
- Considered the individual first and then the organization
- Built a sense of community
- Used a group oriented consensus building decision process
- Provided a frugal approach that conserves resources
- Ability to avoid negative personal or organizational outcomes
- Service profile (500 words max). Provide key dimensions of the nominee's service to the community:
- Length of service with current organization
- Number of youth involved
- Name of teams/programs served
- Date of last background check by organization
- Contact information (phone, address, e-mail address) for:
- Organization submitting the award
- Organizational leader approving the award (include title)
- Nominee
- Statement that nominee will attend the Servant Leadership Appreciation Luncheon on April 25th.
- Servant profile (500 words max). Provide examples of how the nominee demonstrated the value of servitude:
- Letters of recommendation
- Three letters are required
- Preferably prepared by:
- Youth or alumni from program/team
- Parent of youth
- Associate in the organization (board, staff, volunteer, etc.)
- Each letter must include contact information (phone, address, e-mail address) of person preparing the letter.
All letters of nomination must include the following statement: "Our organization and our nominee (provide name) understands that the decision of the selection committee and board are final and we waive our rights to dispute or challenge that decision."
